Rulemaking activity
of license. If the original license or certificate of renewal is lost, misplaced, stolen, or destroyed, the licensee shall immediately report that fact to the board in writing. Upon the board's receipt of the statement by the licensee, verified under oath, detailing to the board's satisfaction circumstances making replacement necessary, the board may issue a duplicate of the original license or certificate of renewal.
