Rulemaking activity
of change of name. If, for any reason, a licensee's name changes, the licensee shall immediately file with the board a certified copy of the legal instrument which makes the change effective or other written evidence authenticated to the satisfaction of the board that a change in name has occurred. The board, upon surrender of the original license or certificate of renewal, shall reissue the license or certificate of renewal in the changed name.
