20:80:20:01Application for plan of supervision renewal – Trainee.

A trainee plan of supervision expires one year from the date of issuance. An application to renew a plan of supervision must be submitted at least thirty days before the expiration of the plan of supervision. To renew a plan of supervision, the trainee shall submit to the board: (1) A completed application; (2) The renewal fee; and (3) Documentation of the trainee's completion of four hours of qualified continuing education focused on ethics. Failure to meet these requirements is grounds to deny renewal of the plan of supervision.
