20:80:20:02Application for plan of supervision renewal – Certified addiction counselor supervisee.

A plan of supervision for a certified addiction counselor supervisee expires one year from the date of issuance. An application to renew a plan of supervision must be submitted at least thirty days before the expiration of the plan of supervision. To renew a plan of supervision, the supervisee shall submit to the board: (1) A completed application; (2) The renewal fee; and (3) Documentation of the supervisee's progress, over the last twelve months, in completing the educational requirements for a certified addiction counselor, as prescribed by § 20:80:15:05. Failure to meet these requirements is grounds to deny renewal of the plan of supervision.
