An ambulance service must provide the department at the time of initial licensing with the name, license or certificate number, and expiration date of the license or certificate, for all ambulance personnel with the ambulance service. An ambulance service must notify the department within thirty days of any change in personnel and provide the name, license or certificate number, and expiration date of the license or certificate, of the personnel.
