IV and Level V trauma hospital designation application. Any hospital applying for Level IV or Level V trauma hospital designation shall submit an application to the department on a form prescribed by the department. The department or its designee shall conduct an on-site visit to verify the content of the application. Once the application is approved, the department shall issue a certificate of designation to the facility. The certificate of designation shall have an expiration date of no more than three years from the date of issuance.
