review -- Documentation. Provider of direct HCB services employees completing the medication administration training are subject to a proficiency review at least annually that is supervised by a licensed registered nurse. The provider of direct HCB services shall maintain documentation of annual evaluations of an employee's proficiency in the employee's personnel file.
