Each provider of direct HCB services must have policies and procedures that address medication errors. The policies and procedures shall address these areas; (1) A definition of medication error; (2) The types or severity of medication errors; (3) Documentation of medication errors; (4) Action to be taken as a result of medication errors; (5) A process for on-going quality assurance and monitoring of medication errors; and (6) Prevention of medication errors.
