The inpatient psychiatric hospital shall assign each minor an identification code consisting of six characters, as follows: (1) The first character is the first letter of the minor's first name; (2) The second and third characters are the total number of letters in the last name, disregarding any spaces within the name; (3) The fourth character is the first letter of the minor's last name; (4) The fifth character is the first letter of the minor's second or middle name. If there is none, it shall be left blank; and (5) The sixth character is used for twins or triplets, with a 9 assigned to the first minor, an 8 to the second minor, and a 7 to the third minor.
