67:42:18:16Maintenance of records.

A kinship foster parent shall maintain a record on each kinship child living in the home. The record must include: (1) The child's name; (2) The date of the child's placement; (3) The date the child is removed from the kinship foster parent; (4) The name and telephone number of the child's social worker, doctor, dentist, legal parent or guardian, and the individual to contact in the event of an emergency; and (5) Any other information required by the department.
