That § 34-20G-88.1 be AMENDED: 4 34-20G-88.1. TheFor each cardholder and nonresident cardholder, the 5 department shall submit the name and date of birth of a qualifying patient who receives 6 a registry identification card to the prescription drug monitoring program, authorized 7 pursuant to chapter 34-20E, the cardholder's: 8 (1) Name; 9 (2) Physical address, except that if the applicant is homeless, no address is required; 10 (3) Date of birth; and 11 (4) Gender; and 12 (5) Registry identification card number. 13 12
